Falls Prevention Program Specialist

The Ashland County Health Department (ACHD) is hiring a per diem Falls Prevention Program Specialist. This position will lead ACHD’s efforts to plan, implement, and evaluate quality improvement initiatives in support of the requirements of the Falls Prevention Program of Holmes, Wayne, and Ashland grant.

Duties and Responsibilities:

  • Participate in A Matter of Balance coaches training.
  • Offer two A Matter of Balance courses in Ashland County within a year of becoming a trained coach.
  • Coordinate and provide site details and appropriate Ohio Department of Health required documentation for A Matter of Balance
  • Regularly participate in the Standing Against Falls Coalition.
  • Complete and submit deliverables as required by the Falls Prevention Program of Holmes, Wayne and Ashland grant.
  • Other duties as assigned by supervisor.

Job Type: Per diem with flexible hours

Education: High school or equivalent (required)

Experience: Previous work with senior citizens preferred.

Work location:  Matter of Balance courses must be conducted in person, however, other job responsibilities could be conducted remotely.
